I hope that you have all had a nice summer. I sure did, and now I’m looking forward to an autumn just bursting with referrals of wills cases! With that in mind, Vanessa and I have revised our referral forms — both the one-page Intake form, available by clicking here, and the Life-planning Documents Questionnaire, available by clicking here. You’ll notice a few changes.
One small but important change is that both forms feature our new Elderly Project fax number — (347) 521-5732. We can still receive faxes on the old 212 number, but we prefer this new fax line, which is only for Elderly Project faxes.
The Intake form now asks the referrer to obtain a bit more information about the potential client’s assets — bank accounts, investments, property, etc. — if what’s requested is assistance with wills and advance directives.
The Life-planning Documents Questionnaire asks for more information in regard to the potential client’s will, as we’d like to obtain as much of this information in advance as possible. Also, a summary of the nature and purpose of each life-planning document appears beneath its respective subheading.
So please keep the referrals coming!
